In order for students to participate in any athletic events, the following forms must be completed and returned before the beginning of any practices. Forms may be returned to Coach Mobley or Coach Miller. Should you have any questions, please contact the SISD Athletic Director's Office at 325-235-3455.
Note: Free Physicals will be given at the Mustang Field House on Saturday, May 19, 2012 from 8am to 9:30am for all incoming 7th and 9th graders. Incoming 11th graders and those with prior major injuries may come from 9:30am to 11am.
PHYSICAL FORM: The student athlete entering the 7th, 9th, or 11th grade MUST receive a new physical for that year. The 8th, 10th, and 12th grade athlete may not need a physical for the school year if they already have an accepted physical on file with the athletic department. The athlete may receive a new physical if desired. The medical history part of the form MUST be completed each year even though a new physical may not be required. Please read the information on the physical examination form and fill it in correctly. Please make sure that all forms are completed and signed by the parent/guardian and the athlete before they are returned to the athletic department.
ACKNOWLEDGEMENT OF RULES/PARENT OR GUARDIAN PERMIT: This form must be signed in case the student athlete is injured during a workout or game. This form and the physical go into the athlete’s file. If the athlete has to go to the hospital because of an injury, a copy of this permit goes with the athlete to help secure medical attention for the athlete.
ATHLETIC INSURANCE: The school district purchases secondary injury insurance that covers the athlete during athletics only. The athlete should have some other type of personal medical insurance. The Classroom Student Insurance Plan offered by the school will not cover U.I.L athletics. The school policy is a co-insurance policy. At the time of an injury, you will receive an insurance claim form that will explain the claim procedure. Remember that you are responsible for all medical bills and that the school insurance company is not required to take any action on any accounts if you do not meet their entire requirements. Please also remember that Sweetwater ISD is not responsible for the payment of any medical bills.
RANDON DRUG TESTING AUTHORIZATION FORM: Due to the new SISD Drug Testing Policy, this form has been added to the Athletic Participation Packet. It must be signed by a parent or guardian and the student athlete. If you would like to see a copy of the district’s Drug Testing Policy, you can find it on our website; www.sweetwaterisd.net, under Parent Resources.
Due to school and U.I.L. rules, the student athlete CANNOT participate in any athletic workout, scrimmage, or game until all required forms are completed and returned to the athletic department. Therefore, all paperwork must be returned before “two-a-days” begin.
